The NC Pediatric Society (NCPeds) is a non-profit membership organization representing more than 2,200 members across North Carolina. Our mission is to empower pediatricians and our partners to foster the physical, social, and emotional well-being of infants, children, adolescents and young adults. Since 1931, we have worked with pediatricians across the state on special initiatives, education, and issue advocacy.
About the Fostering Health North Carolina program:
Fostering Health NC (FHNC) is a statewide initiative led by NCPeds. The program promotes connecting children and youth in foster care with a medical home that has the capacity to meet their special health care needs and promotes the standards of care developed by the American Academy of Pediatrics (AAP). The FHNC project brings together primary care providers, local departments of social services, care management agencies, and others to better coordinate the comprehensive healthcare needs of children and youth in foster care.

The Program Implementation Specialist will be responsible for promoting the medical home model and importance of comprehensive healthcare. Job duties will include outreach to, and collaboration with stakeholders to educate as well as facilitate systems-level change to improve the health and well-being of youth in foster care in North Carolina.

RESPONSIBILITIES:
EDUCATION/TRAINING, SUPPORT ASSISTANCE & IMPLEMENTATION SUPPORT
• Provide training on the medical home model and AAP guidelines to stakeholders including county departments of social services, medical home providers and staff, care management agencies, public health staff, and others.
• Provide training on healthcare topics relevant to children and youth in foster care, including Adverse Childhood Experiences, trauma, and oversight and monitoring of psychotropic medications, and Medicaid access, to a variety of stakeholders.
• Convene stakeholders at county level to provide support assistance and implementation support on the model.
• Support and work with program staff to establish effective implementation strategies and priorities that balance project goals and available resources.
• Identify barriers stakeholders experience and assist in creating solutions.
• Identify new/additional resources needed for implementation through work with local stakeholders.
• Create implementation tools to be used by stakeholder groups across the state.

POLICY & BEST PRACTICE RESOURCES
• Assist program staff, the FHNC State Advisory Team and workgroups, and NC Division of Social Services in developing policy solutions that address barriers encountered in the field.
• Assist NC Division of Social Services with review, revision and implementation of the federally mandated Health Oversight Coordination Plan.
• Work collaboratively with stakeholders to develop, vet and post new and update existing resources for the publically available online resource library, www.ncpeds.org/fosteringhealthnc
• Participate in workgroups with partner agencies to discuss FHNC-related activities to determine best practices and establish new resources and advance implementation tools.

MEETING FACILITATION
• Convene and facilitate meetings and trainings on the FHNC model
• Lead, co-chair, and/or serve on relevant workgroups as needed for the FHNC State Advisory Team.
• Serve as back-up for the Program Director as needed for facilitation of the State Advisory Team and representing the program at external meetings.
• Other duties as assigned

COMMUNICATIONS AND PROJECT PROMOTION
• Communicate progress towards project scope of work and goals to funder, internal team, State Advisory Team members and other partners.
• Assist with maintenance of the FHNC online resource library, making updates as needed to relevant resources and Best Practices documents.
• Share barriers encountered in the field with internal team and State Advisory Team for problem-solving
• Present the project, our goals, and collaborative efforts to partners and potential partners.
• Present the project and project achievements to state and national organizations, when possible (i.e vendor or exhibitor tables)
• Other duties as assigned

TEAM FUNCTIONS
• Participate in all relevant trainings and team/organization meetings
• Assist in recruiting topics and speakers for State Advisory Team meetings
• Travel throughout state in order to perform the essential functions of the role
• Complete reporting requirements to funders on the project

ADDITIONAL QUALITIES OF A STRONG CANDIDATE
• Familiarity with Fostering Health NC program preferred
• Knowledge of NC Child Welfare/Foster Care policy and/or knowledge of Community Care of North Carolina/care management, a plus
• Knowledge of Adverse Childhood Events/impact of trauma/resilience, a plus
• MSW, MPP, MPH, MPA, MBA, BSN or other relevant degree or experience
• Must be able to work collaboratively on small team as well as independently
• Excellent communication skills
• Keen attention to detail and strong, courteous follow-up
• Proficient in Word, Excel, and PowerPoint; knowledge of Zoom, GoTo products, a plus
• Consistent access to vehicle for travel - once safe and appropriate (typically this position requires regular travel
within NC – we expect travel to resume at some point, date TBD)
• Ability to learn new skills and software to perform essential functions
• Ability to work some evenings and/or weekends (staff are periodically called on to participate in NCPeds-related events or partner agency events)
• Reports to FHNC Program Director or designee
• FHNC coverage area is statewide – candidate will need to live in NC and be able to commute to Raleigh regularly
