Transitions LifeCare has been caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness in Chatham, Durham, Franklin, Granville, Harnett, Johnston, Orange, and Wake counties since 1979.
While Transitions LifeCare provides a circle of care to each patient and family member, we do not serve alone. We depend on and refer to other area organizations involved in providing goods and services, such as food, appropriate housing, education, counseling, etc.
Transitions LifeCare is in the process of launching a patient/family behavior policy that gives staff actionable steps if they experience any form of bias. This policy also includes recommendations for the patient/family to ensure they know how to find care from a different organization.
Transitions GriefCare goes above and beyond the Medicare-required grief counseling by providing conversations and workshops, events for children and adults, and online family resources. Transitions CaregiverSupport connects caregivers with area resources and referrals, and we host annual Caregivers Summits to connect care providers with caregivers. Our education department provides talks for professionals and community members to meet them where they are in their understanding of end of life matters.
